Hours: Monday-Friday, 8:00am-4:30pm; nurses are on call 24/7.
Eligibility: Individuals who need medical care in their home following an illness, surgery, or accident.
Fees: Medicare, Medicaid, private insurance and individual payment.
Intake Process: Call for more information or ask your physician.
Service Area: Forest, Langlade, Lincoln, Iron, Oneida, Price, and Vilas Counties.

Program/Service Notes:
The home health team can provide customized care and support to assist the patient to return to independence quickly, safely and effectively in the comfort of their own home. Our health professionals work with the patient's physician to formulate an individualized treatment plan to help patients live as fully and independently as possible. Care is available from registered nurses, physical therapists, occupational therapists, speech/language pathologists, medical social workers, and home health aides.
