Program/Service Notes:
A financial assistance program available to all qualifying patients of Ministry Health Care. Its purpose is to ensure access to health care and assist with financial obligations for medical care provided by Ministry Health Care facilities.
To apply, you will need to fill out a financial form which is available from the Business Office, call the phone number above.
Using that form, a Financial Counselor will apply criteria to determine if you are eligible for a reduction in your bill. The Counselor will contact you shortly after you have applied to let you know the reduction allowed. If you are denied Community Care, you can appeal the decision by contacting the Financial Counselor.
