ADVANCED DIRECTIVES
The hospital provides assistance in helping guide your medical decision making, as well as materials that further explain the Power of Attorney.
COMMUNITY LINK: Information and Referral
Help with financial, medical, chronic health issues, and assistance in connecting with area health providers.
HEALTH ACCESS PROGRAM:
A program to assist with access to medical care for people who are uninsured or underinsured.
DISCHARGE PLANNING:
Timely and effective discharge planning services ensure that patients leaving the hospital have prearranged care available at home, if needed. Long-term care issues can also be addressed.
PALLIATIVE CARE SERVICES:
Palliative Care is the comprehensive management of physical, psychological and spiritual needs of patients and families. The focus is to improve quality of life, emphasizing care of the whole person, including family and loved ones.
