Program/Service Notes:
Palliative care is for patients with advanced or progressive diseases (such as Cancer, Multiple Sclerosis, Congestive Heart Failure, Emphysema, etc.) The Palliative Care Program is available to assist patients and their families and provides a resource of information to the community. Palliative care focuses on relief of suffering and improving quality of life. The goal is to help the patient maintain real life for as long as possible and to insure that there is something to look forward to with pleasure. The person can be receiving active treatment and does not have to be dying. The program also connects people to services in the home (such as Home Delivered Meals, Lifeline or personal care assistance), and assists patients with financial difficulties in accessing available support programs.
