Hours: Monday-Thursday, 8:00am-5:00pm; Friday, 8:00am-12:00pm.
Eligibility: Children ages 12 and under, however must have referral if MA or over 6 years old.
Fees: Private Insurance, Self Pay, and Medical Assistance or BadgerCare.
Intake Process: Call to schedule an appointment.

Children ages 3 to 5 do not need a referral; however, child must be referred by another dentist or doctor if MA and over 6 years old.
