Hours: Tuesday and Thursday, 8:00am-4:00pm.
Eligibility: Children ages 12 and under, however must have referral if MA or over 6 years old.
Fees: Private Insurance, Self Pay, Medical Assistance and Badger Care.
Intake Process: Call to schedule.
