Program/Service Notes:
The mission of the American Academy of Pediatrics is to attain optimal physical, mental, and social health and well-being for all infants, children, adolescents, and young adults.
One of the priorities of the American Academy of Pediatrics is that all children receive care through a medical home. A medical home is not a building, house, or hospital, but rather an approach to providing comprehensive primary care. A medical home is defined as primary care that is accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective.
The American Association of Pediatrics web page includes information on Medical Home Initiative for Children with Special Health Care Needs as well as the other activities and initiatives of the AAP.
Also a resource of information for families and providers on prevention and promoting quality health care.
