Program/Service Notes:
PHYSICIAN SERVICES —
Staffed with a combination of physicians and licensed nurse practitioners. We offer specialty areas which include: Family Practice, Women’s Health/OBGYN, Pediatric, Internal Medicine and Audiology services. We also offer a wide variety of services which include diagnostic and treatment services, medication management, IV’s, casting, stitches, and EKG’s.

LAB AND RADIOLOGY —
We offer a full service, stateof-the-art lab. In addition, we offer general x-ray services as well as ultrasound and FDA certified mammography services on site.

PHARMACY SERVICES —
Available to individuals of
Native American heritage and Potawatomi Community tribal employees only.

DIABETES PROGRAM —
Specialized diabetes education, medication management, foot care assessment,
exercise programming, nutrition management, and nursing support for tribal members.
