Program/Service Notes:
The interdisciplinary team of hospice provides care to the patient as well as their family, offering comfort care rather than curative treatment. This team includes the patient's primary physician, hospice medical director, registered nurses, social workers, spiritual counselors, home care aides, trained hospice volunteers and bereavements support The emphasis of hospice care is on quality and dignity at the end of life. Hospice includes the entire family in the decision-making processes and care. Bereavement support is provided for 13 months after the death of the patient, and longer when indicated.

The Seasons of Life Hospice Home provides a comfortable living environment for individuals receiving hospice care who cannot, for whatever reason, remain at home.

*Hospice care is available to people of all ages, with any terminal diagnosis, in the comfort of their home.
*As part of their mission, Ministry Home Care provides hospice care to everyone who is medically eligible, regardless of their ability to pay.
*Grief support is available to everyone in the community, regardless of their involvement prior to the death.
