Program/Service Notes:
The interdisciplinary team of hospice provides care to the patient as well as their family, offering comfort care rather than curative treatment. This team includes the patient's primary physician, registered nurse, social worker, spiritual counselor, home care aides, trained hospice volunteers and bereavement support. The emphasis of hospice care is on quality and dignity at the end of life. Hospice includes the entire family in the decision-making processes and care. Bereavement support is provided for 13 months after the death of the patient, and longer when indicated.
*Hospice care is available to people of all ages, with any terminal diagnosis, in the comfort of their home.
*As part of their mission, Ministry Home Care provides hospice care to everyone who is medically eligible, regardless of their ability to pay.
*Grief support is available to everyone in the community, regardless of their involvement prior to the death.
*A Time To Mourn, a free grief support group, meets on the 2nd Tuesday of the month , 1:00pm - 2:30pm. Just one meeting per month. Info: 715-356-8805.

Eagle River - "Side by Side Grief Support Group" 3rd Thursday of the month from 1pm. - 2:30pm. at St. Peter the Fisherman Perish.
