Operating Agency: Wisconsin Oral Health Program (Governmental)

Hours: Monday-Wednesday, 8:00am-4:30pm.
Eligibility: Those patients who have PHYSICAL DISABLILTIES, are CHRONICALLY ILL, are 65 and over and they must have inability to pay for needed dental care.
Fees: Generally none; however those with ability to pay are encouraged to do so.

Program/Service Notes:
The Donated Dental Services program is endorsed by the Wisconsin Dental Association (WDA). WDA dentists participate in the program by volunteering to donate their services in their own offices, to medically compromised patients. It is available for those patients who have PHYSICAL DISABLILTIES, are CHRONICALLY ILL, are 65 and over and they must have inability to pay for needed dental care.
