Hours: Monday-Friday, 8:00am-4:30pm; On-Call staff 24/7.
Eligibility: People of any age who have weeks to months to live and choose to focus on care, on symptom and pain control.
Fees: Services are covered by most health insurance, including Medicare. Fees vary.

Program/Service Notes:
Offers hospice, palliative and grief care, when active treatment is no longer the focus of care for an illness. Hospice care concentrates on relieving symptoms and supporting patients and their families. Care is provided for people in their homes, in the hospital and in other living facilities. Provides and ensures meaningful and compassionate end of life care. Provides a special kind of care aimed at helping people live as comfortably as possible.
