SERVICES INCLUDE:
PHYSICIAN SERVICES -
Family Practice, Internal Medicine,Women’s Health,
Pediatrics, Physiatry and Podiatry. We also offer a
wide variety of general services which include diagnostic
and treatment services, medication management,
IV’s, casting, stitches, and EKG’s.
REHAB SERVICES -
Physical Therapy and Speech Pathology services are
available 5 days/week for pediatric through adult.
Massage Therapy is available on a limited
schedule.
OPTICAL SERVICES -
Our optometrist can provide for your eye care
needs. We have a storefront where you can purchase
fashionable frames, lenses, sunglasses, sport
safety frames and contact lenses.
DENTAL SERVICES-
Our dentists, orthodontist and two hygienists are
available to serve patients of all ages.

and Potawatomi Community tribal employees only.
Community Health Services -

• These services are provided to Potawatomi Tribal
members and other Native American individuals
(depending on the service).These include in-home
nurse and community health representative visits,
nutritional services, parenting education, domestic
violence support and counseling, infant services,
immunizations, elder services,WIC program, medical
transport, diabetic counseling and programming.
LAB SERVICES-

We offer a full service, state-of-the-art lab. We also perform
occupational drug urinalysis. Lab testing can be
performed by the order of Forest County Potawatomi
physicians and outside providers. This allows you to have
frequent lab draws done close to home.
RADIOLOGY SERVICES-
715-478-4339
We have on site state-of-the-art x-ray and ultrasound
equipment. We offer the only mammography
services available on site in Forest County.
BEHAVIORAL HEALTH SERVICES -

State certified outpatient mental health & alcohol
drug services. We offer:
• Psychological evaluations and psychometric
testing.
• Mental health psychotherapy on an individual
basis, family therapy, couples & group therapy.
• Alcohol and drug assessments, referrals to
inpatient treatment, intensive outpatient
treatment for adults and adolescents.
After-care groups for men and women.
• AODA prevention for children and adolescents.
• Mental health & AODA services for adults.
• Art Therapy children & adolescents.
The Health & Wellness Center in Crandon, Wi, was built to provide medical, dental, behavioral health and pharmaceutical services, optometry, physical therapy, speech therapy and community health services to tribal members, other Native Americans, and residents of the surrounding communities. The 42,000 square-foot center is open to the general public and provides many services which are not available anywhere else within a 30-mile radius.
